I read a fic a while back where HG was pregnant with Sev's twins. The pregancy was difficult, and Harry jumps to conclusions that Sev is cheating on Hermione and there is a confrontation that she gets in the middle of, she gets accidently pushed, or falls from the moving staircase and loses one of the babies.

I'm almost certain that the story you have in mind is "Vengeance Is Sweet" by southern_witch_69 -- I came across it when I was looking through some of the reviews I've left on Ashwinder. (Oddly enough, it also turned out to be the story which has been on my mind recently but which I couldn't remember the name of, one in which Hermione learns that her parents originally married one another out of convenience and friendship rather than love.)
